The Infected Gum Sandwich is a special item that allows the player to infect the Black Ant Queen when fed to it. It can be unlocked by finding the Infected Gum Sandwich Recipe, which can only be found in the Black Anthill Lab in New Game+. Specifically, it is found in the room above the Assistant Manager arena full of parts of damaged MIX.Rs.
When fed, the Black Ant Queen will die and cause all the same effects as the Poison Gum Sandwich, though with a small notice appearing saying "You sense a ripple in an alternate dimension". When the player starts New Game+ or goes to the next New Game+, the Black Anthill will become infected, with Infected Ants and Infected Soldier Ants replacing some of the Black Worker Ants and Black Soldier Ants. Along with this, the Black Ant Queen will become an Infected Ant Queen.
It can only be used once, and using it locks the player out of feeding the Black Ant Queen any other options of Gum Sandwich for that save as well as the next New Game+, due to turning the Black Ant Queen into an Infected Ant Queen.
