The Infected Mushroom Sandwich is a special item that allows the player to infect the Red Ant Queen when fed to it. It can be unlocked by finding the Infected Mushroom Sandwich Recipe, which is found in the Exposed Pipe that leads to the Haze Lab, specifically directly below the entrance to the lab.
When fed, the Red Ant Queen will die and cause all the same effects as the Poison Mushroom Sandwich, though with a small notice appearing saying "You sense a ripple in an alternate dimension". When the player starts New Game+ or goes to the next New Game+, the Red Anthill will become infected, with Infected Ants and Infected Soldier Ants replacing some of the Red Worker Ants and Red Soldier Ants. Along with this, the Red Ant Queen will become an Infected Ant Queen.
It can only be used once, and using it locks the player out of feeding the Red Ant Queen any other options of Mushroom Sandwich for that save as well as the next New Game+, due to turning the Red Ant Queen into an Infected Ant Queen.
