The Infected Toejam Sandwich is a special item that allows the player to infect the Fire Ant Queen when fed to it. It can be unlocked by finding the Infected Toejam Sandwich Recipe, which is found in the Fire Anthill in New Game+2. It can be found via taking the Lawnmower entrance and traveling forward. It will be found on the floor of a fairly open food room not too far in the cave.
When fed, the Fire Ant Queen will die and cause all the same effects as the Poison Toejam Sandwich, though with a small notice appearing saying "You sense a ripple in an alternate dimension". When the player starts New Game+ or goes to the next New Game+, the Fire Anthill will become infected, with Infected Ants and Infected Soldier Ants replacing some of the Fire Worker Ants and Fire Soldier Ants. Along with this, the Fire Ant Queen will become an Infected Ant Queen.
It can only be used once, and using it locks the player out of feeding the Fire Ant Queen any other options of Toejam Sandwich for that save as well as the next New Game+, due to turning the Fire Ant Queen into an Infected Ant Queen.
